Exam Verification

This exam verification form is for an employer seeking exam verification information on an O.D. for employment. NBEO® develops, administers, and scores the entry-into-the-profession examinations that are used by regulatory boards of optometry in making licensure decisions. In order to request exam verification information, the employer must also submit the NBEO Release Form signed by the candidate.

The NBEO-Board Certification Program was discontinued on February 1, 2019 due to low participation. As a result of this, verification of previous Board Certifications are no longer available.

NBEO does NOT maintain information regarding the licensure status of optometrists.

NBEO will verify the following:

  • The examinations taken
  • The month & year the examinations were taken
  • The pass or fail status (“P” or “F”)

Exam Verification Fee: $60.00

The verification will be electronic to the email address used at the time of purchase and may take up to two business days, once NBEO recieves a complete request form.

Important Message

NBEO is aware that some candidates have been informed by their state boards that score access is unavailable. Since March 2026, NBEO has provided all state boards with the ability to register for and access the new State Board Portal, along with multiple communications, onboarding instructions, and training resources. If your state board indicates they do not have access, we encourage them to contact NBEO directly for assistance so we can support completion of their portal setup.
